Advice on Work Capability Form for ESA due in Wednesday

When filling in the section 'Please tell us about your illness and disability' how much of the history of my illness should I put in?

My problems started back in the 1990's. I was misdiagnosed for years which caused me to have to give up work in 2001 because I could not manage with what turned out to be not stress but AS, and other health problems. I have been too unwell to work since due to a combination of these problems (living off my retirement savings).

Should I put the history e.g. dates of registered blind, eye surgeries, consultations with nurologists (chronic daily headache). Details of failed medication due to side effects e.g. cyclosporin.

I only have a couple of days to get this in and am shaking and frozen with indecision.

Thank you for any advice you are able to give.
