Hello
I was just wondering what kind of forms would I need to apply for esa? Do I need to get a gp note aswell as evidence from rheumatologist ( i.e letters)

    go to www.gov.uk
    and have look at all the infomation there.
    yes all information from rheumatologist and doctor will help you
    in applying for esa.
