Early retirement

Hi I am 64 I have rheumatoid arthritis. I wondered who is best to talk to about early retirement due to ill health. I can't really finish with out financial help.what happens is when I get a flare up it affects my hands and shoulders which isn't good as I am a van driver. Work are really good as they understand my predicament and don't complain if I take a few days of now and again
