I contacted my local Social Services and they sent me the form to complete. You need photos too. Once completed, photocopy the form so you have a record of your answers and return the original to them. Mine also require a cheque for £2.50p. I gave them permission to contact my rheumatologist. They take between 6-8 weeks to come through. Note the expiry date, and apply for your renewal at least two months before that . DDHave you got the despatches? No, I always walk like this. I think there are two ways of applying for a BB.
If you already have DLA at the higher rate mobilty...then the form filling is simple. Where I live (Scotland) social services manage the BB scheme. The form is quick to complete as you only require to fill in one section... attach a copy of your DLA entitlement.. and two passport sized photos (one for their files and one goes on the BB) took two weeks from start to receiving the badge
If you don't have DLA ... you have to complete all sections of the form and provide contact details of your GP/ rheummy. it takes a bit longer as your doc or rheummy has to provide info to support your application.
good luck!

I got refused but they asked for further documentation from my consultant - which took 6 weeks to arrive!! So make sure you send them copies of any letters with your diagnosis etc. I did send them but they obviously couldn't read. Still waiting.0

Yep! I did. The assessment was a joke. In there for about 3 minutes. She was ticking the boxes before I even answered the questions. I offered documents but she only took one. They said they didn't have my diagnosis - it was on the letter I gave them and also sent. They repeated that I had to be in severe pain when I walked, which I'd said. Wanted to make a formal complaint but I got very ill with a chest infection that laid me low for a few weeks. Only just surfacing and dealing with things. If they refuse me again I am going to my MP. Watch this space.......!0

Hi,
I think it depends where you live as it used to be Dr and social services but now the council have taken over in my area so you need to get the form from them its free aswell.
so i would ask your council first.

Hi
I recently completed the form on line and paid the £2.00 administration on line as well. I received an email stating that I was likely to get it and I printed off the form I had to sign and sent my photo in two weeks ago.
Still waiting for it to come. The email address is local.direct.gov.uk
Hope this is enough info for you.
