Hi all
Can you tell me who would give best help with wording/completing a dla form ? whos have you found the best? CAB? the councils welfare person? a person representing the charity of the condition you suffer from?
thanks.

    Hi Oneday,

Nice to see you by the way and hope you and your friend are both doing ok?

I don't know the answer cus unfortunately it is down to how good the person you see is more than who they represent if that makes sense? DIAL are good for advice and do help fill in the forms, as do the CAB but both (this way) have very long waiting lists now a days.

I know the NACC people have a dummy form for the IBD and that i did think was really good but i didn't think I was bad enough with the crohns to use it......

You might be able to try one and if you feel its not right or helpful see one of the others.

I'm not really helping flowr but i just don't know cus it is, to a degree, down to the individual you see.

One things for sure its best to have help though. Rally hope someone else will know for sure the answer and hope you are doing ok? Cris x

    Hi Oneday,
Just thought I'd say, I applied in August for DLA, I had the forms for ages as I was collecting evidence to go with it, I did not get advice from anyone I did it myself. The evidence I supplied was: a report of my condition and it's prognosis from my Rheumatologist (I informed them I was applying and asked would they do me a report, I had to wait 3-4 wks but I got it and they didn't mind) a prescription list of all the medications I am on from my GP (who was the person to recommend I applied for DLA in the first place!) a copy of all the adaptions I've had to make to my home to support my disability as well as a report from my Occupational therapist. Because I broke my wrist I was sent for a bone scan which showed bone density reduction so I sent a copy of that, a copy of all the letters and appointments ive had from Rheumatology/Physio/Hand Physio/pain clinic etc and a copy of my Blue Badge acceptance letter.
I got the reply back in 2 weeks with a confirmation of DLA. With all the evidence to support my claim that I am suffering from a debilitating chronic condition, as well as reports from the professionals involved with treating me, I think I did as much as I could to prove my case. It just goes to show you don't always need to go to a charity to help you fill out the forms. Wishing you a painfree day X

    Hi Poppy
I think once I put my mind to it like you did gathering evidence I should manage to complete it. I printed off a blank form and keep making notes.
Was your rhemys report used for the part where you have to ask someoen to write someting about you or did you ask friend or relative?
thank you, you are getting my brain cells thinking-jump starting them!!

    Hi Elizabeth

time to think- i am counting down the days for that, when school is back! Im surprised i have gone out properly dressed lately....my head had been pounded with hyper kids for summer hols. The only peace i get is the drive to work!
The charity info on line is very good and got me sort of started to make some notes, then i printed a dla form on line and tried to match my notes to the questions.
Once the kids are back my head might not be as cabbaged to be able to think straight.

    Hi,

I paid £15 (it's £19.45 now) and got access to info with examples of how to fill it out and explained how to answer the questions in three stages. The three stages they explain are brilliant as you are answering possible questions before they get chance to ask you them, it has both sections incapacity and mobility and also help and advice for appeals. It was brilliant, i spent a while doing it, all I also got a letter from a collegue at work, letter from my husband, copy of prescription and sent it away. Within a few weeks I was given the highest rate for both so well worth it would really recommend investing the money in it.

Let's not forget though why you are applying, for help due to this disease taking things away from you and making it easier for you. I have AS which is a chronic disease and I would swap all the DLA money to not have AS. I used mine to get an automatic car and help me in other ways.

Good luck however you apply. Xx

The way they show younto do it is great I think it was something like say what you can't do, what it stops you doing and then why using what they suggest wouldn't help. For example I struggle to teach full time but having a seat would not help as I would not be able to do my job something like that if you get what I mean. It's trying to pre-empt what they will ask and be one step in front really worth doing it.

I printed out a blank copy and went through the notes and made notes on each question then went through each question with the examples and put me into that situation. Really important to write as much as you can, even if you have to use more paper. If I can help PM me. Xx
