DLA -Quick Question

Hi All,

I hope your all well, quick question, I got a phone call from my doctor on Friday he said he had recieved a letter about my DLA claim and has asked me to go and see him tomorrow so we can fill in the form together in the hope of getting my DLA claim approved, I just wondered if you guys knew of anything or had any advice on what he should put in order to help with my claim.

    Hi Julie

    He needs to list diagnoses l think and would help if he added

    all meds all consultants you see, any physio etc you have, pain clinics etc

    Any aids he knows you have (zimmer etc), your ability to work as he sees it and pognosis length of time you have had your illness/es

    Of course any actual questions he is asked.

    That's a start anyway.

    When I applied I listed my then GP as a contact but I have no idea whether he was contacted or not. :oops: I think frog has given you a good check list and I hope this leads to a successful claim. Good luck and please let us know how you get on as your experience will, I am sure, help others. DD

    I'm not sure, but its nice that your doc cares enough to help you. Good luck, fingers crossed that all goes well.
