Anyone had to fill ESA 3 form in?

Just received an ESA 3 form in post today and was wondering if anyone else had filled one in?

    Hi, I haven't had to fill one in but I understand it's relating to moving from contribution-based ESA to income-based ESA, if that fits. It's a long form I believe.

    From your other thread (update on medical), is that the form you are referring to on there; I assumed before that you would have already filled in the ESA50 before being called for the medical. Just checking whereabouts you are in the whole thing..
