Has anybody had help filling in their ESA50 form?

Asking on behalf of a relative who hasn't had a form yet but has had a letter about ESA and just wanted to ask if anyone sleeked some help with this and what was it like.
Or is it better just to fill it in and see what they come back with, This person has bad MS and struggles with bad nerve twitches and severe muscle weakness and has had a leg removed because of gangrene and they also have kidney failure and undergo regular dialysis
Would they ask them to attend a medical, As it seems everyone has to have one even after filling in the ESA form.
They are in their late 50`s and find travelling very difficult.

Hi,
I filled my hubbys in ... It hurt my hands & wrists badly!! :?
I'd suggest that they ask their Dr to contact DWP about having a home medical. We did this & wen the GP lost her temper at their initial refusal they soon agreed!!!
Most charities, such as the MS Society, will be able to advise who can help with actually filling in the form.

Thanks Debbie, All we know is that to expect the form soon. Its a nightmare as when all this esa stuff is done they are doing the DLA replacement with more forms and medicals, it must be costing a fortune to do.
When they ask for medical evidence are we just better letting them obtain this from the relivent doctors and speciaists or do they require you to actually provide the evidence from these people.

Umm, for ESA they get it themselves from the GP but I'd be prepared for PIP ... I rang my dermatologist & rheumatologists secretaries & said I needed a report of my overall condition, they both did me proud & I was awarded 5 yrs highest rate for both, DLA. That was a couple of months ago so I scrapped in b4 PIP but the reports worked brilliantly. They think we're all liars now but believe our Drs, They can't really argue with them xHealing Hugs

if you take form to the CAB they will actually fill it in for you, also get as many letters from GP, consultants as you can, I hope your relative gets through without needing an assessment.................Marie xxSmile a while and while you smile

It's all just a nightmare and stressful time with all this changing over and reassessing!! As I've said in another topic about it I've come out in bad eczema because of me stressing over it. My OH filled it in for me and we both answered the questions and photocopied the forms so we have a copy of what was filled in. I've put as many reports I've found copies of I have with the forms and given details of my G.P., rhumatologist and Rhummy nurse so they can contact them. All we can do is wait for the outcomes.0

we got our carers support worker to help us fill in the forms + got letter from consultant to our GP
not everyone gets called for a medical so do your homework well & hopefully you wont need to & get put straight into the support group , just remember you need to score 15 points on one part ie walking , to score 15 points overall you will end up in the working group0 -
I he DIAL help me with filling my forms in.I was put in the support Group last november.Dont be fooled by being put in that group you WILL be re assesd i have just received more forms to fill in even though i was put in support group.How much more stress are we meant to go through0
