In need of template for doctor to complete

Hi everyone! I'm new to this forum but as I've looked through topics, I'm reassured to have found a great community and hope to contribute as well as ask questions.

I'm an American and having to file for a disability claim through the USA Social Security system. My Rheumatologist does not understand what is needed for submission so that I can be approved. Does anyone know of a form I can access and use as a sort of template for him?

I have Psoriatic Arthritis, Fibromyalgia with documented joint damage. Diagnosed initially in 1988 and in treatment including biologic meds since that time.
