Anyone heard of an ESA-3?

I rang the JC re my ESA appeal yesterday. I was concerned that it was taking so long (I sent it July 2012). As my ESA contribution based finished 2nd February and my retirement pension starts 5th March I didn't want them to just shove me at the back and hope I went away.
I feel I should have been in the WRAG group and would like the back pay that I consider I am due.
The lady on the phone told me that I needed to fill in an ESA-3 to get money for the month between 2nd February and 5th March.
I had never heard this mentioned before so wonder if it something they keep under wraps unless it is asked for (as they seem to with some benefits).
Just a heads up for any of you that may be in a similar position.

    I've heard its about 50 pages long. Make sure you get proof of posting as they are very good at losing things and you don't want to be submitting a 50 page form twice!
